Our Impact

As of September 2019, we have provided 2,000 primary care visits for over 750 patients. We are seeing especially high needs and volume from people displaced by violence in Syria.

Our application of the Patient Medical Home Model: 

Since opening, our partnerships and engaged executive team have enabled our patients to receive not just primary medical care, but also services from Allied Professionals working with patients within the same space.

7 different types of interdisciplinary team services are available to our patients on premise, thanks to our dedicated team and generous support from our partners. We are continuing to expand our on-site interdisciplinary services.

Special Activities in 2018

In our first year of opening, we

  • Established a direct referral process with the local health authority’s New Canadian Clinic, which previously had difficulty transitioning some patients into appropriate community-based primary care. 

  • Built bidirectional referral partnerships with established community agencies, including Muslim Food Bank's Aspire program, DiversCity, Options, and Success. Our medical team provided education at community events in partnership with these organizations, with a focus on health system access for newcomers and the appropriate utilization of health services in Canada.

  • Became a participating member of the BC Multi-Agency Partnership to share up-to-date, interdisciplinary resources for refugee claimants with a network of agencies serving our population.

  • Engaged with the boards of Community Health Centres across metro-Vancouver to share learnings and participate in developments in global funding led by the BC Association of Community Health Centres

  • Dialogued with and received official written support from the BC Ministry of Health in regards to the value of Community Health Centres and our specific work in Surrey. 

  • Presented to the Fraser Health Authority Board of Directors about our operations and model. 

A Strong Start

284 community members signed a petition to open a Community Health Centre in Surrey for vulnerable newcomers. After opening, we had over 560 patient visits in our first 36 clinic days.
