Apply to become a member of Roots Community Health Centre Society! As a member, you become an official part of our care community.

Becoming an approved member is simple, and shows your support to the community we service. If you want to be more involved with Roots CHC Society, membership will enable you to have the privileged opportunity to attend and take part in our General Meetings, to serve on Committees where invited, and to participate in membership programs.

Applying to be a member simply requires that you are 19 years of age or older, and a $5.00 CAD annual membership fee. Click Apply below!

Please note: We are a registered non-profit society, and though we are working to obtain Charitable Status, tax receipts are not available for membership dues or donations.
