We are moving!

Roots Community Clinic is moving to a larger and more convenient clinic!

The new clinic aims to open on April 1st . Please do not go to the new location before April 1st. If there is a delay in opening, we will email patients.

The address is 8630 Scott Road (120 Street), Surrey. The clinic is located at the back of the building with parking in front of the Community Health Centre and in the adjacent parking lot.

Roots CHC phone number remains the same. 604-498-3565.

IMPORTANT: Between March 1st and April 1st all appointments will be on the phone.

We look forward to continuing to serve you at our new enlarged clinic.
